Corporation Profile Report for Health Professional Corporations

Almost all of the colleges (Royal College of Dental Surgeons of Ontario, College of Physicians and Surgeons of Ontario etc.) require a Corporation Profile Report. There may be a very few that still require a Certificate of Status. If you are unsure which report type is required please check the renewal information provided by your college before proceeding below.
